RESUMO

Objective:To analyze the risk factors of carbapenem-resistant Klebsiella pneumoniae (CRKP) infection in intensive care unit (ICU) patients and to construct a prediction model for infection. Methods:The clinical data of 204 patients with Klebsiella pneumoniae infection admitted in ICU of Jining First Hospital during January 2020 to December 2022 were retrospectively analyzed. Patients admitted during January 2020 to December 2021 were selected as model set ( n=150), and patients admitted during January to December 2022 were selected as validation set ( n=54). In model set, there were 59 cases infected with CRKP (CRKP group) and 91 cases infected with carbapenem-sensitive Klebsiella pneumonia (CSKP group). The risk factor of CRKP infection in ICU patients were analyzed with multivariate Logistic regression, based on which an infection prediction model was constructed. The predictive value of the model was evaluated by ROC, and verified in the validation group. Results:Multivariate Logistic regression analysis showed that empirical use of beta-lactam antibiotics( OR=6.985, 95 % CI 1.658-29.423, P=0.008), central vein catheterization( OR=7.486, 95 % CI 2.776-20.186, P<0.001)and tracheal intubation/incision( OR=10.695, 95 % CI 2.701-42.351, P=0.001)were risk factors for CRKP infection in ICU patients. The regression equation for predicting the risk of infection was -4.851+ empirical use of beta-lactam antibiotics×1.944+ central vein catheterization×2.013+ tracheal intubation/incision×2.370. The area under the ROC curve (AUC) of the model for predicting infection in the model group was 0.905, with sensitivity and specificity of 79.7% and 90.1%, respectively. The AUC of the model for predicting infection in validation group was 0.881, with sensitivity and specificity of 84.2% and 85.7%, respectively. Conclusion:The constructed infection prediction model in the study can effectively predict CRKP infection in ICU patients.

RESUMO

Neuroblastoma(NB)is the most common extracranial solid tumor in children.It is notable for highly heterogeneous and associated with tumor histologic classification and differentiation status, with ganglioneuroma representing fully mature and differentiated NB.Differentiation therapy reduces the adverse reactions caused by treatment without affecting normal cells and tissues by inducing the redifferentiation of NB cells, and has a good development prospect in the maintenance treatment of high-risk NB patients.Therefore, studying the key molecules and signaling pathways affecting NB differentiation is significant to further clarify the pathogenesis and improve the prognosis of neuroblastoma.This article reviews the important molecules related to NB cell differentiation, signaling pathways and the research progress of differentiation induction therapy.

Objective To investigate the effect of interfering with E2F3 gene expression on the invasion and migration of prostate cancer cells and its mechanism. Methods The expression of E2F3 gene in human prostate cancer Du145 cells was knocked down by siRNA. The cells were divided into three groups: control group, Du145 NC group (siRNA-NC)and Du145-siRNA group(siRNA-E2F3). Cell migration and invasion were detected by Transwell invasion and wound healing assay. The expressions of E2F3, E-cadherin and MMP-9 proteins were detected by western blotting. Results After transfection, the expression of E2F3 protein in the Du145-siRNA group was significantly lower than the control group(P< 0.01). The number of invasive cells and wound healing rate of Du145 cells in the Du145-siRNA group were significantly lower than the control group(P < 0.01). Furthermore, the protein expression of E-cadherin was significantly increased(P< 0.01)while MMP-9 decreased(P< 0.01)in the Du145-siRNA group. Conclusions E2F3 silencing can inhibit the invasion and migration ability of prostate cancer Du145 cells, and this might be accomplished by regulating E-cadherin and MMP-9 protein.

Objective To investigate the effects of simulated microgravity by rotary cell culture system (RCCS) on expression profiles of miRNA in mouse fibroblasts L929 cell line.Methods L929 cells were cultured in vitro and divided into simulated microgravity (SMG) group and normal gravity (NG) group according to the simple random method.Samples of two groups were collected on 7th day of culture and the total RNAs were extracted,labeled,and hybridized in sequence.Feature Extraction Software was used to collect the array images and get raw data,which were analyzed by Genespring Software.Differentially expressed miRNAs were identified and then validated by qRT-PCR.Target genes of differentially expressed miRNAs were predicted by the databases of Targetscan and microRNAorg,and the intersections of databases were identified as potential regulatory target genes.Gene ontology (GO) and Kyoto encyclopedia of genes and genomes (KEGG) analysis were applied to determine the roles of these target genes.Relevant biological functions and/or signaling pathways of the regulated genes especially related with wound healing process were categorized based on their enrichments.Then integration predictions of the miRNA and mRNA expression profiles had been proposed to refine the functional miRNA-mRNA relationships.The miRNA-mRNA functional network and miRNA-mRNA control network were constructed.Results Four miRNA genes were up-regulated significantly including mmumiR-669j,-122-5p,-30a-3p,-6516-3p,among which mmu-miR-669j was up-regulated at 52.84 folds with the greatest significance (P < 0.05).Seventeen miRNA genes were down-regulated significantly including mmu-miR-21a-3p,-miR-28a-5p,-218-5p,-210-3p,-miR-19a-3p,-miR-31-3p,and-miR-19b-3p,among which mmu-miR-28a-5p was down-regulated at 15.47 folds with the greatest significance (P < 0.05).The qRT-PCR showed a high concordance with the microarray results (P < 0.05).Target gene prediction and functional enrichment analysis suggested that a variety of biological processes and signaling pathways involved in wound repair were significantly enriched (P < 0.05).Function network and regulation network of miRNA-mRNA covered all the differentially expressed miRNAs,which suggested that miR-21 a-3p and predicted target gene Smad3 might play an important role in wound healing under microgravity.Conclusions Simulated microgravity by RCCS can significantly affect the expression of stress-related miRNAs in mouse fibroblasts L929.The miRNA target gene prediction and functional enrichment analysis based on gene chip technology may provide theoretical basis for illustrating the mechanism and management of weightlessness stress injury.

Objective: To monitor vancomycin plasma concentration results by clinical pharmacists in order to participate in individualized medication adjustment.Methods: Clinical pharmacist monitored the blood concentration of vancomycin in special patients, and then designed the individualized medication according to the monitoring results and clinical manifestations of patients.Results: Through the individualized medication adjustment, clinical pharmacist enhanced the efficacy and reduced the incidence of adverse reactions, and improved the level of clinical rational drug use as well.Conclusion: Clinical pharmacist can provide better service for clinics through the practice of individualized precision pharmacy, and reasonable and safe pharmacy service can be provided for patients.

RESUMO

Objective To study the effect of telbivudine (TBV) on the ratio of CD4+ CD25+CD127low/-regulatory T cell (Treg) of HBeAg-positive chronic hepatitis B (CHB). Methods The ratios of Treg before and after treatment for 12, 24, 36 and 48 weeks with TBV of thirty-five patients with HBeAg-positive CHB were detected. Results Ratios of Treg were (12.55 ± 1.78)%, (10.42 ± 1.04)%, (9.55 ± 0.74)%, (8.78 ± 0.67)%, (8.74 ± 0.71)%respectively before and after treatment of 12, 24, 36 and 48 weeks and all the values were lower than its former one (P < 0.05), except the 48 weeks′. Conclusions The ratio of Treg of HBeAg (+) CHB decreases after treatment with TBV.

Aim Toconstructthreeplasmidsincluding Smad3 WT,Smad3 EPSM and Smad3 3S-A stable transfection in HepG2 cell lines to investigate phospho-domains of Snad3(pSmad3C or pSmad3L),their pro-tein expression and roles in HepG2 cell proliferation, apoptosisandcellcycle.Methods Threeplasmidsin-cluding Smad3 WT (Carry the wild Smad3 gene ), Smad3 EPSM(Carry the mutated phosphorylation site in linker region of Smad3 gene)and Smad3 3S-A(Car-ry the mutated phosphorylation site in C-terminal of Smad3 gene)were respectively transfected into HepG2 cells by using a liposome transfection reagent.Verifi-cation of positive cells was done by screening with G418 via co-culture.Transfection efficiency was deter-mined by Western blot.Cell proliferation was induced by exogenous TGF-β1 in the respective stably transfect-ed HepG2 cell lines.Cell proliferation was monitored by MTT.Cell cycle and apoptosis were determined by flowcytometry(FCM).Results Therewaselevated protein expression of the respective phospho-domain sites in the stably transfected HepG2 cells for Smad3 WT(C-terminus and Linker),Smad3 EPSM(C-termi-nus)and Smad3 3S-A(Linker),which indicated suc-cessful stable transfection of HepG2 cell lines.The re-sults from MTT experiment showed that TGF-β1 could induce proliferation of HepG2 cells with or without the transfection of Smad3 WT,Smad3 EPSM and Smad3 3S-A plasmids,meanwhile transfected Smad3 EPSM plasmids could significantly inhibit proliferation of HepG2 cells induced by TGF-β1 , and transfected Smad3 3S-A plasmids accelerate proliferation of HepG2 cells induced by TGF-β1 .Cell cycle analysis showed that the G0/G1 phase of HepG2 cells with stable trans-fection of Smad3 EPSM plasmid increased compared with HepG2 cells with or without stable transfection of Smad3 WT plasmid,meanwhile the G2/M phase of HepG2 cells with stable transfection of Smad3 3 S-A plasmid increased.Compared with Smad3 WT trans-fected cells, apoptosis in Smad3 EPSM transfected cells was markedly increased,while that of Smad3 3S-Atransfectedcellsdecreased.Conclusions Thethree plasmids of Smad3 WT,Smad3 EPSM and Smad3 3S-A stably transfected HepG2 cell lines have been suc-cessively constructed.The construction of three plas-mids transfected HepG2 cell lines provides the research foundation for studying medical as well as possible reg-ulatory mechanism of pSmad3 C/pSmad3 L.

Spillage of cyst contents during surgical operation is the major cause of recurrence after hydatid cyst surgery. Instillation of a scolicidal agent into a hepatic hydatid cyst is the most commonly employed measure to prevent this complication. SB202190 is a pyridinyl imidazole derivative and is known to be a specific inhibitor of p38 MAPK. In the present study, the scolicidal effect of SB202190 was investigated. Freshly isolated Echinococcus granulosus protoscolices were subjected to SB202190 treatment (10, 20, 40, and 80 microM), and the effects on parasite viability were monitored by trypan blue staining. Corresponding effects were visualized by scanning and transmission electron microscopy. Dose-dependent protoscolex death within a few days of SB202190 treatment was observed. Although the in vitro scolicidal effect of SB202190 was satisfactory, the in vivo efficacy of this drug and also possible side effects remain to be further investigated.

The professional care by multi-disciplinary team and priority of prevention should be carried out in the treatment of diabetic foot disease to reduce diabetic amputation.This article describes the professional experience in the treatment of four complicated cases with diabetic foot disease and emphasizes the importance of the co-operation among different specialists,including diabetologists and wound,vascular,orthopedic surgeons,etc.as well as of varied therapies applied in staged management of the diabetic foot care,by treating these patients with diabetic foot disease as early as possible.

Objective To investigate the antimicrobial susceptibility of Pseudomonas aeruginosa (P. aeruginosa) isolated from Zhenjiang area to 13 routinely used antibiotics and identify the structure and dissemination of class Ⅰ integron. Methods K-B test was used to determine the resistant rate of 71 strains of P. aeruginosa. DNA template was extracted by boiling method, PCR method was utilized to detect class Ⅰintegron, and subsequently gene cassettes were analyzed by sequencing. Results The resistant rates to 13 routinely used antibiotics were quite different from 18. 3 to 77.5% among 71 strains of P. aeruginosa. The prevalence of class Ⅰ integron was 38%. These integrons include 5 gene cassettes ( aadB, aac (6) - Ⅱ , PSE-Ⅰ , dfrA17 and aadAS), in which dfrA17 and aadA5 gene cassette were frequently found. Comparing with the negative strains of integron, the positive strains of integron has obviously higher resistance to ten the antibiotics including piporacillin, piperacillin-tazobactam, ceftriaxone, cefepime, ceftazidime, gentamicin,amikacin, tobmmycin, levofloxacin, and ciprofloxacin. Conclusions The resistant rates of P. aeruginosa to 13 drugs were different, and the resistant rates of integron positive strains were obviously higher than integron negative strains, which indicates that integron may play an important role in multidrug reisistance of P. aeruginoosa.

Objective: To study the mechanism of safflower injection(红花注射液) on preventing thrombosis after arteriovenous fistula operation in rabbits.Method: Twenty-four rabbits were randomly divided into three groups: safflower injection group,heparin group and control group(each n=8).Rabbit femoral arteryvein side-to-side anastomosis models were performed on both sides of the lower extremities and they were coincided.Safflower injection(5 mg/kg) or heparin injection(6 U/kg) was given from ear edge vein immediately after operation in the respective group,while normal saline(0.9%) was given with the same fashion in the control group,once per day for 14 days.On the 1 st,3 rd,7 th and 15 th day after operation,the nitric oxide(NO) in serum and endothelin(ET) in plasma were determined respectively.Results: The plasma ET contents were significantly lowered in safflower and heparin groups compared with that of control group(both P

AIM: To explore the influence of herbs (9602 prescription) on brain energy metabolism in cerebral ischemia reperfusion mice.METHODS: Ischemia reperfusion in cerebral injury model was duplicated in mice. Using nuclear magnetic resonance (NMR) and high performance liquid chromatography (HPLC), the brain metabolism were measured. The influence of 9602 prescription on cerebral energy metabolism in ischemia reperfusion mice model was dynamicly observed. RESULTS: Phase Ⅰ: The spectrum of NMR showed that after 10 min of ischemia, the PCr peak dropped significantly, while the Pi peak rose significantly in both the control and the “9602” group. There was no remarkable difference between the two groups. After reperfusion the PCr peak in the control group continued dropping slowly and remained at a low level (55.50?14.94) after 10 min of reperfusion, while after reperfusion the fallen PCr peak in the “9602” group started rising till 76.72?13.37 (P0.05). AIM: To explore the mechanism of Nao-re-qing oral liquid (NRQ) decreasing endotoxin (ET)-induced fever in rabbits. METHODS: (1) The ET-induced fever model was established in rabbits. Febrile response of rabbits was observed. AIM: To investigate the effect of Zhongfeng Nao De Ping, a traditional chinese medicine, on the cerebral ischemia/reperfusion injury in rats. METHODS: Delayed neuronal death (DND) model was established by bilateral carotid arteries occlusion for l0 min followed by 1 day and 7 days reperfusion. Effects of sodium glutamate, ketamine and Zhongfeng Nao De Ping on DND in the hippocampus was examined by counting neuronal density in CAl Sector, and the contents of Ca 2+ and amino acids in the hippocampus were also measured. RESULTS: The contents of the glutamate and aspartate in the hippocampus were increased significantly at l0 min after transient cerebral ischemia. Sodium glutamate markedly decreased the neuronal density in hippocampal CAl sector, while ketamine and Zhongfeng Nao De Ping increased it significantly in the same sector. The level of Ca 2+ increased on day 1, day 7 after the reperfusion, while Zhongfeng Nao De Ping inhibited the increase in Ca 2+ concentration in hippocampus. CONCLUSION: These results suggested that Zhongfeng Nao De Ping has an effective protection against DND in hippocampus following cerebral ischemia/reperfusion in rats.

AIM:To compare the effects of alcohol and aqueous extracts from Huiyang shengji gao(HYSJG),a Chinese medicine,on proliferation of human fibroblasts derived from normal and ulcer margin skin.METHODS:Using explant culture techniques,we obtained populations of normal human fibroblast(nHFB)and ulcer marginal fibroblast(uHFB).Alarmar blue assay was used for observing cell proliferation,and flow cytometry for cell cycle analysis.RESULTS:After cultured with aqueous extracts and alcohol extracts of HYSJG for 24 hours,the proliferation rates of the two cell lines were both elevated.For alcohol extract,obvious proliferation-promoting concentrations on nHFB and uHFB were 0.027-0.425 mg/L and 0.027-0.213 mg/L,respectively(P
